(138) Trends in All-Cause Mortality for Older Adults With Ruptured Aortic Aneurysms in the US (1999-2020)

Introduction: Aortic aneurysm rupture is a rare and potentially fatal condition. Recent studies have shown a decreasing prevalence in the UK, Sweden, and the US. Screening for aortic aneurysms and closer follow-up, along with improved techniques for repair, have led to a decrease in the number of patients who present with ruptured aortic aneurysms.

Methods: A retrospective analysis was conducted using the Centers for Disease Control and Prevention Wide-Ranging Online Data for Epidemiologic Research (CDC WONDER) dataset. Patients above the age of 65 with mortality from ruptured aortic aneurysms were identified using the ICD-10 codes 17.1, 17.3, 17.5, and 17.8. Age-adjusted mortality rates (AAMR), crude mortality rates, and annual percentage changes (APC) were calculated through Joinpoint regression. The results were stratified to uncover the temporal, demographic, and geographic disparities in AAMRs.

Results: From 1999-2020 there were 113,094 deaths from ruptured aortic aneurysms occurred in older adults. The overall Age-Adjusted Mortality Ratio (AAMR) was 12.37 per 100,000 people. The AAMR has decreased dramatically from 23.23 to 5.99 per 100,000 individuals. The Average Annual Percentage change(AAPC) was -6.31%, p < 0.05. For males, the AAPC between 1999 and 2020 was -7.01%, 95% CI -7.39 to -6.81. For females during the same time period, the AAPC was -5.34, 95% CI -5.57 to -5.15. All Races showed a reduction in mortality from ruptured aortic aneurysms, there were no obvious race-related disparities observed. Morality also showed a consistent decrease among all census regions. Northeast [AAPC -6.21%, 95%CI -6.52 to -5.99], Midwest [AAPC -5.69%, 95%CI -6.04 to -5.48], South [AAPC -6.37%, 95%CI -6.91 to -6.01], West [AAPC -6.49%, 95%CI -6.88 to -6.16].

Conclusions: There have been significant improvements in all-cause mortality from aortic aneurysm rupture in older adults. Widespread screening, reduced tobacco consumption, and endovascular and surgical repair improvements have likely contributed to these mortality benefits.
