The management of mechanical ventilation after the initiation of VV ECMO differs substantially from typical approaches to lung protective mechanical ventilation. While lung rest may have its benefits, recent evidence calls into question the optimal approach to mechanical ventilation, recognizing that dense atelectasis may prolong or complicate eventual separation from ECMO.
