Survival rates of in-hospital cardiac arrest (IHCA) remain poor, with high morbidity among survivors. Evidence guiding IHCA management is limited and driven by extrapolated guidance from out-of-hospital cardiac arrest (OHCA). This session will address differences between IHCA and OHCA, including patient population and causes of arrest. Speakers will discuss diagnostic and therapeutic options and the benefits and risks of intra-arrest transthoracic and transesophageal echocardiography toward diagnostic and management approaches during arrest. Calcium is a commonly administered yet nonrecommended therapeutic option. It may be beneficial in IHCA because hyperkalemia and hypotension are more common etiologies, but short- and long-term harm has been reported after use in OHCA. Extracorporeal membrane oxygenation CPR in OHCA as applied to IHCA will be explored, as will post-arrest management with a focus on temperature therapies, cooling, and elevated temperature after IHCA. Speakers will discuss neuroprognostication after arrest, including IHCA etiologies and timing of neurologic assessments.
